Services:
We create a specialized care plan for each client. This care plan will directly address health care needs without requiring repeated travel from home to hospital.
The point of home health care is to enable you or the client to stay where you prefer - usually to stay home - and still receive the medical care you require.

Home Health Aide:
Performing appropriate and safe techniques in personal hygiene and grooming that include:
Safe transfer techniques and movement around the home with or without equipment
Normal range of motion and repositioning
Adequate nutrition and fluid intake
Simple dressing changes
Reading and recording temperature, pulse, respiration and blood pressure
Catheter care and changing of catheter drainage bags
Colostomy care
Assistance with diabetic testing care
Meal prep., light housekeeping, laundry, marketing and business errands
Respite care
Performing appropriate and safe techniques in personal hygiene and grooming that include;
Bed bath
Sponge, tub or shower bath
Shampoo (sink, tub, or bed)
Nail and skin care
Oral hygiene
Toileting and eliminiation

Skilled Nursing:
Our team of registered Nurses (RN) are experts in the management of patient care in the home. Whether you are recovering following a recent stay in the hospital, learning abut and dealing with a new diagnosis or trying to manage a chronic disease, our RN's will be there to assist you and your family through the entire process. Nursing care can include but is not limited to:
Education related to the diagnosis
Evaluation and monitoring of a condition
Medication management
Injections
Wound Care
Blood draws for evaluation and treatment of an illness
Post surgical care and recovery
Care and maintenance of catheters, feeding tubes and IV's

Personal Care Aide:
PCA services are offered to qualified individuals who need support and assistance withe every day activities in order to be more independent in their home environment and community. PCA's provide your services under the supervision of a Reliable Home Healthcare, LLC Registered Nursed. These services include:
Bathing, grooming and hair washing necessary for personal hygiene
Turning, positioning and transferring
Medication reminder
Dressing and undressing
Assistance with food, nutritition and diet activities
Skin, bowel and bladder care
Range of motion exercises
Short errands
Assistance with therapeutic activities


Private Duty Nursing:
Private Duty Nursing services provide assistance with the day-to-day tasks of life - 24 hours per day, 7 days per week. Care is provided for those who need hospital level nursing to maintain independence at home. These services are available for people of all ages, from your new born baby to a spouse or grandparent. Our nurses are trained for complex cases including:
IV therapy & treatment
Ventilator care
Trachea care
Feeding care
Catheter care
Wound care
Many different therapies
Personal cares
Any other care needed and prescribed by your doctor

Occupational Therapy:
The goal of Occupational Therapy is to help you regain the ability to perform everyday activities independently, such as eating, dressing, bathing, preparing a meal or enjoying your favorite hobby. Your Occupational Therapist will help you to:
Adapt your day-to-day routine to meet you abilities
Improve fine motor skills so that buttoning a shirt, putting on shoes or cutting food becomes easier
Live safely comfortably in your own home
